Documents Required
A comprehensive checklist of mandatory documents needed for processing your medical reimbursement claims efficiently and without delay.
Identity & Basic Forms
- Duly filled Claim Application Form.
- Employee/Beneficiary ID Card copy.
- Aadhar Card and PAN Card copies.
- Cancelled Cheque for bank details verification.
- Dependence Certificate (if claim is for a family member).
Hospitalization Records
- Original Discharge Summary.
- Hospital Final Bill (Original) with detailed breakdown.
- Payment Receipts from the hospital.
- Doctor's Prescription advising admission.
- Operation Theatre (OT) notes (if applicable).
Pharmacy & Diagnostics
- Original Pharmacy Bills supported by prescriptions.
- Diagnostic Test Reports (Blood tests, X-Ray, MRI, etc.).
- Original Payment Receipts for lab tests.
- Implants/Stent invoice with sticker (if applicable).
Certificates & Appendices
- Essentiality Certificate 'A' (for OPD) or 'B' (for IPD) signed by AMA.
- Emergency Certificate (if admitted in a non-empanelled hospital).
- Non-Availability Certificate for medicines.
- Referral Letter (if referred to another hospital).
Important Note
Always maintain an additional photocopy set of all the documents submitted for your personal records. Ensure all original bills are stamped and signed by the respective authorities.
